1. My motivation and vision for ESR

I have been actively involved in ESR activities for many years, serving as a member of the ECR Program Planning Committee, as Chairperson of the Chest Subspecialty, and more recently as Chairperson of the Communication and Interdisciplinary Affairs Committee. Radiology is a rapidly evolving field that has driven significant advances in medicine, both in diagnostics and therapeutics. It was this dynamism that inspired me to pursue radiology during my time as a pediatric resident, where my clinical experience shaped my understanding of its impact.

We are currently witnessing a period of profound change in healthcare, and I would like to share my vision for the strategic decisions that will shape the future of our specialty.

  • As artificial intelligence (AI) increasingly integrates into the field of imaging, it is crucial to emphasize the role of radiologists—not merely as technicians but as physicians in their own right. Radiologists play a pivotal role by synthesizing clinical, biological, and imaging data to arrive at accurate and timely diagnoses. Their work requires direct communication with patients and collaboration with other healthcare professionals. The European Society of Radiology (ESR), through its Patient Advisory Group, has been actively working on improving communication practices for radiologists of all experience levels. The development of diagnostic consultations by radiologists, for secondary analysis of imaging files, particularly for complex cases, will increase our visibility to the public.
  • To enhance visibility within the medical community, it is essential to conduct independent research, especially in fields where imaging plays a central role One such area is cancer screening through imaging, which is why I have obtained funding from the French Ministry of Health to conduct a lung cancer screening study in women, evaluating the role of AI as an alternative to double human reading.
  • Increasing specialization within radiology is essential to ensure that our analyses have a meaningful and lasting impact on patient care. As medical imaging becomes more advanced, radiologists must go beyond surface-level interpretations to provide insights that directly influence clinical decisions and treatment plans. This requires not only technical expertise, but also a comprehensive understanding of disease clinical manifestations, their progression, potential complications, and available treatment options. The ESR-endorsed subspecialty diplomas (Level III) formalize advanced expertise, equipping radiologists with the specialized knowledge necessary to tackle complex cases and enhance their role in multidisciplinary care.
  • At the same time, maintaining cross-disciplinary expertise is essential, particularly for emergency diagnoses. Through the ESR’s ongoing educational programs, radiologists can continue to sharpen their skills across multiple disciplines. This broad knowledge, combined with our 24/7 availability, positions radiologists as indispensable players in healthcare systems. Optimizing imaging techniques and evaluating new technologies are responsibilities that radiologists, with their unique expertise, are best suited to undertake, further solidifying their essential role in healthcare
  • Radiological expertise is particularly valuable in multidisciplinary settings—not just in oncology, but across many areas of medicine. Radiologists should be financially compensated for the time they devote to multidisciplinary team meetings, taking into account the number of cases that require radiological review or reinterpretation. The investment of time in such meetings directly contributes to better patient outcomes and cost-effective care. The concept of value-based radiology must be emphasized and expanded to demonstrate the true impact of radiology on patient care.
  • Looking ahead, it is imperative to train the next generation of radiologists to work effectively with AI. Detection algorithms and automated quantification tools will enhance the reliability and reproducibility of diagnoses. These tools will run systematic evaluations in the background, saving time and enabling more personalized assessments for patients. My department has PACS-integrated AI solutions for fracture detection, assisted reading of chest X-rays and detection of lung nodules on CT scans. For the moment, these solutions are available for testing, but we'll have to build the economic model that will enable all radiologists to invest in this field.
  • The expansion of interventional radiology techniques as an alternative to traditional surgery and in emergency situations is essential, and is an additional motivating factor for new generations of radiologists.
  • Finally, the ESR, through its various subcommittees—covering research, education, e-health, AI, and quality and safety—will continue to promote an innovative and forward-thinking radiology, capable of addressing future challenges.

1. My experience in the field and my motivation

Close colleagues describe me as a highly motivated and most honest radiologist with a very strong dedication to clinical work, basic research, mentoring and teaching. After almost 17 years at Stanford University, I recently returned to Europe and now I would like to contribute to our Society in best and most dedicated way possible. I am convinced that I could make a tremendous impact as chair of the educational committee given the deep insights into education I had in my career on both continents, US and Europe.

In 2006, I gave the ECR New Horizons Lecture as a newly board-certified Radiologist, just before I moved to Stanford University for a postdoctoral fellowship in molecular imaging. Even during a time dedicated to basic science laboratory work, I made sure to attend as many clinical radiology teaching events as possible. It was eye-opening how experts of every radiology subspecialty interpreted radiology exams, what insight they could extract out of exams in Stanford. It was due to a well-curated team of organ-based sub- specialized and modality-lead radiologists. It was very clear to me that Stanford Radiology was an exemplary residency and fellowship program. In 2009 I joined the faculty of the Stanford Radiology Department, learning the ropes of educating the next generation of radiologists from some of the giants in radiology teaching. They did not shy away from adopting novel technology to increase their teaching outreach even further, such as Youtube teaching channels, medical zoom conferences long before that was adopted elsewhere. During my time at Stanford, the department was awarded the AuntMinnie.com

“Best Radiologist Training Program” award twice, a highly coveted distinction among academic institutions in the USA. Education was a cornerstone of that department and taken extremely seriously. To this date, I admire the rigorous training system, including fellowship training. Later, as division chief of musculoskeletal imaging, I was deeply involved in shaping the curriculum of residents and fellows in our program, helping the MSK division become the most coveted rotation for our trainees. I am proud to say, that the Stanford curriculum was and still is one of the best recognized in the US. Furthermore, I served hands-on as a direct mentor to medical students, radiology residents, and basic sciences post-doctoral students for many years and actively contributed to the highly successful CME course program of Stanford Radiology. Thus, the needs of the to-be-trained are well familiar to me, allowing me a good judgement in structuring education.

Fast forward to now, after returning to Europe last year, my experience has been that radiology in Europe has evolved very much. I am deeply impressed by the quality of the presented work and educational efforts at every conference of ESR and their subspeciality societies. I very much appreciate the structure of the ESR Diplomas and applaud the required qualifications. While we will always need generalists in the field of radiology in the appropriate setting, the future of radiology lies in training also subspecialized experts to provide the most value in patient care. Due to many wise actions in recent years, the ESR is extremely well positioned in the online teaching world. Education is the main foundation of Radiology’s future. To attract the best talent, we need to start with outreach early, in medical schools and even high schools to bring the field of radiology to the forefront of awareness in the younger generations. In the domain of continuing education, I am a big proponent of exchange among the different cultures of radiology in the form of targeted conferences, travel fellowships, direct outreach, and with an even further radius through online learning. We can all learn so much from each other. The ESR is fostering that, and I want to do my part in investing in the future of Radiology.

1. My experience in the field and my motivation

I consider my previous experience working for ECR/ESR (mentioned below), and my experience on a national level, to be an added value to better fulfill the position I am now applying for. Working for ESR/ECR has been a true pleasure and a wonderful chance to learn and to share experiences with radiologists all over Europe and the world. It has been, at the same time, a challenge and a source of fulfillment to work towards the improvement of our society and the harmonization of radiology in Europe.

It is my belief that I can serve the society in a responsible and contributive way. Moreover, my candidacy also takes into account the need for a good geopolitical fit, ensuring that Portugal, a country that usually ranks high in the number of attendees and submitted abstracts for ECR, is represented at the European level.

I sincerely have a strong commitment towards my professional tasks and undeniably believe in the advantages of teamwork. Therefore, I consider that I possess the personal conditions (including availability of time) to fulfill my duties and tasks related to the position I am now applying for. In this way, I envisage this as a perfect opportunity to contribute to the growth of our society and, ultimately, of radiology.

In this regard, promoting the ESR’s educational, research, and professional policies in member countries will be pivotal, as well as ensuring a functional liaison between ESR and national authorities, bodies, and professional organisations.

1. My motivation and vision for ESR

With 15 years of dedicated research in cancer imaging biomarkers, I am committed to advancing precision medicine through the integration of cutting-edge imaging techniques. My journey began with pioneering work in whole-body MRI for bone metastasis response evaluation at the Royal Marsden Hospital in London. Now, as the leader of the Radiomics Group at VHIO (Barcelona), my focus is on multiparametric MRI studies and applying AI models to extract insights from medical images, driving innovation in cancer diagnosis and treatment.

Motivated by the potential of imaging biomarkers to transform patient care, I am passionate about expanding this research approach within the European Society of Radiology.

One of my priorities is to encouraging young radiologists and radiographers to pursue academic careers as essential point to bridging the gap between clinical practice and cutting-edge research. My goal is also to build bridges between disciplines, accelerate knowledge transfer, and bring the latest advancements in imaging to clinical practice, benefiting both health care providers and patients.

As Chair of the ESR Research Committee, some of my objectives will be to:

1. Foster collaboration across disciplines (radiologists, other physicians, radiographers, engineers and industry partners) to accelerate knowledge and bring breakthroughs to clinical practice faster.

2. Organize joint workshops focused on interdisciplinary research skills, innovative imaging technologies, and translating research into clinical practice.

3.Stablish further collaborations with pharmaceutical companies and biotechs, incorporating imaging biomarker studies into clinical trials.

4. Enhance the competence and knowledge of radiologists and radiographers in training by promoting research opportunities and facilitating access to grant applications and collaborations, particularly within European-funded consortia.

2. Major Leadership positions at international level

As the head of the Cancer Core Europe (CCE) Imaging Task Force, I spearhead radiomics research within the consortium and collaborate with the Data Sharing Task Force to develop platforms for secure imaging data sharing across CCE centres.

My work extends to editorial contributions, as I serve on the boards of NPJ Precision Oncology, Cancer Research Communications, and ESMO Real World Data and Digital Oncology. These roles allow me to stay at the forefront of imaging research, while also supporting knowledge dissemination in these rapidly evolving fields.

I have led several investigator-initiated imaging biomarker trials, including iPROMET (ClinicalTrial.gov ID NCT05078151), PRECIMET, PREDICT, MARION, and ATHECA radiomics study, collaborating closely with hospitals across Europe.

Beyond Europe, I was awarded the Prostate Cancer Foundation (PCF) Young Investigator Award in 2018 and led the PCF Imaging Working Group from 2019 to 2021.

I am also member of the RECIST Working Group, contributing to defining global standards for cancer response assessment.

1. My motivation and vision for ESR

ESR has focused on leading radiologists into a future in which they become more visible, provide more value, engage more in multidisciplinary teams and increasingly interact with patients. I would like to continue to pursue and expand these ideas together with ESR and help further strengthen the foundations of our discipline to ensure that radiology becomes even more relevant in the decades to come.

I see substantial threats from increasing workloads, financial pressures, radiological practices being bought by investors, decreasing work satisfaction, and the rapidly evolving role of AI that brings hope of better quality and less stress but also the threat of ultimately replacing radiologists as we know them.

During my chairmanship of the Dutch Radiological Society, we succeeded in defining a common vision for the future, in which we see radiologists moving away from mainly writing diagnostic reports towards more important tasks that will ensure that our role remains important even if AI takes over more and more of the reporting process.

I would like to use my experience in this process to build on the existing programs of ESR, refining the common vision for European radiologists so that it can become a blueprint for national initiatives. This vision should involve partnering with other diagnostic specialties to claim a more central role in multidisciplinary teams, and exploring a diagnostic gatekeeper role that ensures that unnecessary steps are eliminated from the healthcare pathway and that patient receive the most appropriate treatment. This requires direct interaction with patients but also adequate reimbursement for tasks that are new for us radiologists.

Adequate scientific proof of new workflows may be necessary to change guidelines accordingly and to ensure that health insurers and governments realize that radiology is able to avoid downstream cost and provide more cost- effective care pathways with better outcomes. I would like to see ESR stimulate European cooperation to more quickly demonstrate the effect of strategically important changes in clinically pathways.

An example is the breast cancer pathway, in which radiology can claim the full diagnostic workup: some 90% of women will not have breast cancer and do not need to see another specialist. Radiologists can provide immediate feedback to their patients, reducing their anxiety and saving time and money in the process. For those with cancer, radiologists can claim the full staging workup together with pathology and genetics, making sure that all information is available once the patients are presented in the multidisciplinary team meeting to define further treatment. New screening techniques, such as breast MRI or dual energy mammography hold the potential to eliminate lymph node metastases from the 2nd screening round onward, opening up opportunities for minimally invasive treatment by radiologists. Other examples are prostate cancer, lung nodules and chronic chest pain, to name but a few. Making sure that these radiology-driven workflows are more cost-effective, potentially also with a better outcome will open the way for reimbursement and a stronger role of future radiologists.

Since we are the medical specialty most involved with AI, we should use this fact to claim a central role as data specialists. However, most radiologists are insufficiently trained to do so and are overwhelmed by clinical work. Gaining back time to become more active outside the pure reporting business is therefore a priority for our specialty. AI can be a tool to make us more efficient, although current systems are insufficiently able to do so. I see a role for ESR to provide guidance for radiology departments and AI companies alike to help us gain time to claim new roles.

In the past decades, we have focused on training organ radiologists that are able to be adequate partners in multidisciplinary teams. We have been very successful in these aspects but have neglected the technical aspects of our field. Training of residents and radiologists alike will be needed to create more radiologists that fulfil the role as data specialists but also can guide industry in new technical developments, where at the moment, industry-push and not radiology-pull is prevalent. ESR has the means to play a pivotal role in this process.

My motivation for applying for this position is to help strengthen our field and to make sure that working in radiology stays rewarding now and in the future.
